This book is a point-of-care resource for effective sexual and reproductive healthcare for patients of all ages, sexual orientations, gender identities and medical backgrounds in the primary care setting. This useful guide is divided into three parts, and other than part three, which deals exclusively with transgender and gender diverse patients, all content will relate to patients of all gender identities.

Part one presents sexual and reproductive health (SRH) using a lifespan approach, including chapters on pediatrics, adolescents and young adults, adults, and older adult patients. Part two presents an approach to common SRH issues that span multiple age groups, including contraception and family planning, sexually transmitted infections and cancer screenings as well as sexual and reproductive health in the setting of common medical conditions. Part three is dedicated to sexual and reproductive health for transgender and gender non-binary patients, including psychosocial, medical, surgical and legal aspects of health.

This book provides primary care clinicians with a framework for providing effective sexual and reproductive healthcare to patients of all ages, sexual orientations and gender identities in a way that is inclusive, focuses on health, and addresses the needs unique to specific populations.

Description: This book is a practical guide for primary care clinicians to use as a resource for sexual and reproductive health (SRH) issues. It is subdivided into three parts. The first section gives a lifespan perspective of SRH that spans from pediatrics to older adulthood and all of the stages in between. The second section deals with special considerations pertaining to high-yield SRH topics, including but not exclusive to family planning, sexually transmitted infections, sexual dysfunction, and pregnancy. The third section focuses on SRH care for transgender and gender non-binary patients.
Purpose: The purpose of this book is to provide a practical resource for primary care clinicians when addressing their patients' sexual and reproductive health. The book prioritizes providing evidence-based guidelines and information on the topics. It pays special attention to how to provide inclusive, non-judgmental, evidence-based care to all patients regardless of their age, gender, or sexual orientation. It provides guidance on how to care for historically marginalized populations including transgender and gender non-binary patients. The book achieves its purpose, but it would be even better with more figures, tables, and diagrams to illustrate the key points. The authors could also have added a bulleted summary at the end of the chapters with the key points. The "conclusion" paragraphs are bland and not high yield.
Audience: The audience is primary care clinicians at every stage of training. The authors have a broad range of expertise. Multiple specialties are represented including board-certified physicians in family medicine, OB-GYN, urology, internal medicine, pediatrics, and psychiatry. Some authors are physicians and nurse practitioners who are very active in the care of LGTBQ populations. At least one of the authors is a transgender woman, which adds credibility to that section from my perspective.
Features: Overall, this book is well written and easy to read. It has some repetition that is within reason considering it is not meant to be read from front to back but instead as a reference like "UpToDate." The references cited are current and appropriate and the majority of authors have actively cared for the conditions and populations covered. This is not a book that I would carry in my white coat pocket, but it would be valuable for students, residents, and practicing doctors to review parts of it. I could especially see its value in preparing for a standardized patient encounter addressing any of the topics covered in the book, both for the person writing the case and for the learner being evaluated. A feature that stands out about this textbook is its coverage of care of transgender or gender non-binary patients and how to take a sexual history. The way I was taught to take a sexual history, which is still how the students are being taught locally to ask about it, is outdated per this text. In addition, the book offers very practical scripts and phrases on how to say things that won't be misconstrued as we care for all people including those who have been marginalized. A weakness of the book is that there are minimal figures, diagrams, and tables. If its goal is to be a reference, distilling the key points into the above would be helpful. The American Family Physician Journal is an excellent example of how to do this well.
Assessment: This book will be helpful at every stage of practice. Many books focus solely on the facts but fail to also address how to implement the information. This book does a good job of addressing the "art" of how to interact with patients on some very complex and sensitive topics while remaining evidence based. I found some of the material to be practice changing. For example, how to obtain a sexual history has evolved over the years and the script provided in the book is extremely helpful.
