Green and Krueter are the authorities
This book is a classic in the health promotion field. The Precede-Proceed model has been around for a while and has evolved over time. The book has good overview of the model and breaks it down in subsequent chapters. There is a good set of chapters at the end on the practical use of Precede-Proceed. I used this as part of the public health PhD qualifying exams study. There are criticisms of this model, however, but obviously you'll have to look elsewhere to find them.
