Purpose: The purpose is to provide insight into what information is important for an ambulatory patient, how to collect that information, and how to design and manage a patient care plan based on the information gathered. With an evolving emphasis on outpatient care and the pharmacist's increasing role in this setting, these objectives are extremely worthwhile and pertinent.
Audience: This book is written for pharmacists who are or will be practicing in an ambulatory care setting. Pharmacy students taking an ambulatory care rotation may also find it beneficial. The American Society of Health System Pharmacists (ASHP), a leading pharmacy organization, published this reference and received contributions from numerous well-respected individuals in pharmacy and ambulatory care.
Features: The manual is divided into three sections: collecting and organizing patient data, developing a plan of care, and managing that plan. In the first section, the reader is instructed how to gather subjective and objective information, and the second section gives insight on how to assess drug therapy, identify therapeutic goals, and design and monitor a therapeutic plan. In the final section, the reader is instructed on how to gather, compare, and evaluate new data and modify the care plan if needed. This learning tool is supplemented with a sample medical chart, forms, and case studies to help facilitate learning. Although the various forms are important for learning, they may not be practical in a real setting.
Assessment: The instruction manual is an excellent learning tool and provides a solid foundation for pharmacists interested in practicing in an ambulatory care setting. The unique approach teaches the necessary skills that a pharmacist will need to provide pharmaceutical care.
