The book is organized by disease processes and covers the major histopathology features of each entity with mentions of ancillary techniques, immunohistochemical studies, and molecular testing as necessary.
Purpose: Lung biopsies present a unique challenge to practicing pathologists with a limited amount of tissue, small sample size, and, at times, unrealistic expectations from clinicians regarding the diagnostic yield from such small biopsy specimens. This book addresses these specific issues by discussing many common concerns, identifying several possible pitfalls, and clarifying what needs to be mentioned for further clinical evaluation and treatment.
Audience: The intended audience is general pathologists confronted with the task of interpreting lung biopsies, but this book is also a great tool for residents and fellows and even a quick reference for clinicians for understanding the processing and limitations of biopsy specimens. It is a good starting point when working up challenging biopsy specimens. The authors are experts in the field.

Suggestions from the authors include how to sign out inflammatory and interstitial lung disease in a way that addresses the heterogeneity of many of these lesions and the specific limitations that a biopsy poses to definitive diagnosis in many cases. Primary and metastatic neoplastic chapters address the need for, and limitations of, immunohistochemical markers in the evaluation of neoplastic conditions and incorporate the significance of new molecular markers in the evaluation of primary lung cancers. The illustrations are of high quality and provide good examples of the major histologic features with descriptive captions to emphasize the findings. Tables are well organized and are particularly helpful in summarizing and comparing information in the infectious disease chapters. Interstitial lung disease is always a diagnostic challenge. The information on these disease processes is well covered in this book, with specific insights into the specific challenges a biopsy specimen presents. A table comparing the pertinent points of each entity (age, heterogeneity, pattern, CT findings) would have been a nice addition to this chapter to summarize the findings. I found myself spending much more time on this chapter, flipping back to earlier entities to organize and compare the different findings. Another area that can be of great confusion is molecular studies for lung cancer. A description of the molecular markers and their clinical significance appears in the epithelial lung neoplasm chapter, but additional discussion of how the authors triage specimens for ordering markers (only stage IV vs. instances where ordering markers on a lower stage lesion would be clinically indicated) would also be of help. Also, it would be helpful to know what algorithm they prefer (EGFR followed by ALK vs. EGFR and ALK concurrently vs. KRAS followed by EGFR and ALK) for time- and cost-effectiveness of molecular results.
Assessment: This is a valuable book on the evaluation of pulmonary pathology that provides vital insights into the specific limitations and challenges of biopsy specimens.
