As patients live longer medical comorbidity has become exceedingly common with various psychiatric disorders, including major depression and bipolar disorder. Indeed, some of our primary treatments for these disorders may actually lead to this comorbidity because of biological consequences of specific pharmacology agents. Thus, today we can no longer easily separate medical and psychiatric disorders in our approaches to specific patients. Indeed, a wider knowledge of the interface is needed for everyday practice. The first edition of this book was an instant "must have" for all psychiatrists, whether they treat patients with comorbid medical conditions or not. The second edition of the Manual is even better, updated and another "must have" book for all psychiatrists. The evidence supporting the use for or against a medication in a particular medical illness is reviewed succinctly. I particularly liked the Key Points at the end of each chapter as they provided a summary of the take home messages. Congratulations to the authors and editors for producing such a valuable textbook.

The book is a useful tool for the busy practitioner because it synthesizes a large amount of information in an easy-to-read manual with clear explanations.

Description: This second edition of a book on psychopharmacological management of patients with comorbid medical illnesses has been updated to reflect the most recent FDA guidelines related to drug dosing, schedules, and routes of administration. The first edition was published in 2010.
Purpose: The purpose is to provide a practical clinical manual and educational tool for specialist and nonspecialist physicians who provide psychopharmacological treatments to the medically ill. Given recent government-based reforms that incentivize the integration of medical and psychiatric care to improve outcomes and reduce long term costs, this is a worthy objective.
Audience: The intended audience is residents, fellows, and practicing physicians. It is not directed to any specialty, as the authors note that many psychiatric patients are in the care of primary care or other physicians. Also, the contributors hail from a variety of different specialties, and most have academic appointments. The utility of this book is evidenced by the number of ACGME-approved fellowships that have incorporated it into their curriculum for use as a core reference.
Features: The first part of the book deals with general principles of pharmacokinetics and pharmacodynamics, as well as basic review definitions related to metabolism of drugs, etc. The second part focuses on psychopharmacology, organized by organ system. A separate chapter is dedicated to severe drug reactions, discussing not what medications to use for an ailment, but what ailments can be caused by specific medications. Each chapter contains numerous visual aids, including tables, diagrams, and key summary points. These are particularly useful for organizing a dense volume of information into a quick reference comparison. The only shortcoming of the book is the lack of accompanying online resources, as many young physicians preferentially refer to online resources.
Assessment: This is a particularly useful tool for those in medical training. It is well organized by topic, with clear explanations and uncomplicated language. It also provides sufficient background information for each topic, starting with very basic concepts, making it ideal for those at the beginning of residency training. The charts and diagrams throughout the book are probably the strongest features. I tend to prefer online resources for day-to-day practice, given the ability to easily access them and the (possibly false) belief that they are more relevant or up to date than print resources. However, I will keep this book out, as its appendixes and tables are superior to what I have found in commonly used online sites. Also, the relevance to pharmacokinetic interactions and quick comparison to other medications would both require a more time-intensive search online to get a similar depth of information. I would highly recommend this book to anyone treating patients with psychiatric and medical comorbidities.
