Description: This book reviews the pathophysiology and management of fluid, electrolyte, and acid-base balance.
Purpose: The purpose is to update the field in light of new developments, despite the common misconception that there remain few new clinically pertinent changes. The book concludes with an integration of the material by analysis through 10 case presentations.
Audience: It is clearly targeted for those desiring a deeper understanding of fluid, electrolyte and acid-base disorders. Nephrologists, nephrology fellows, and any physician with an interest in the area of fluid disorders would benefit from this book. The frequent inclusion of animal models and translational research would appeal to physiologists as well. Due to the high level of obligatory baseline knowledge in fluid disorders and cellular physiology required to understand the bulk of this book, this would not be an ideal introductory book for internal medicine residents or medical students unless they have an interest in pursuing nephrology as a career.
Features: The first four chapters review the relevant basic physiology of fluid and electrolytes. The fifth chapter is an overview of the medical management of pediatric patients complicated by electrolyte or acid-base disorders. The sixth chapter discusses diuretic therapy in the context of the specific pharmacological agent and later in the setting common medical ailments such as cirrhosis, congestive heart failure, and acute kidney injury. Five subsequent chapters build on basic acid-base physiology and discuss each major acid-base disorder in detail. The final chapter summarizes the material with 10 clinical scenarios followed by brief discussions. The case discussions and diuretics chapters are particularly informative. I found the inclusion of animal models and research in the electrolyte chapters intriguing, but would have appreciated a summary at the end of each chapter describing the most salient points and advances in research as they relate to clinical practice. The length of the book is manageable and the dimensions make it portable, but that comes with its own constraints. The font is small, particularly with images and tables. At several points, a figure or table is referenced several pages away, making it difficult to navigate. The text would have been enhanced with more color in figures and tables.
Assessment: This is a valuable resource for nephrologists that also could be useful for internists or researchers with a particular interest in nephrology.
