Willy Baranger weist darauf hin, dass der Begriff des Narzissmus eines der problematischsten und unklarsten Definitionen der psychoanalytischen Theorien ist. Er unterteilt die unterschiedlichsten Bedeutungen von Narzissmus in drei Hauptgruppen ein. "Die erste Gruppe bezieht sich im wesentlich auf den Narzissmus als eine Form der Triebschicksale. Die zweite Gruppe akzentuiert das Objekt in den narzisstischen Stadien, und die Probleme des Narzissmus fallen mit jenen der Identifizierung in ihrer introjektiven Form zusammen. Die letzte Gruppe besteht aus Erweiterungen des Begriffs zur Bezeichnung von Einstellungen, Gefühlen und charakteristischen Zügen, die die Bewertung, Entwertung und Überbewertung eines Aspekts der Person andeuten."
Jede Begriffsgeschichte des Narzissmus beginnt entweder mit den psychoanalytischen Vorläufern Freuds, Sadger und Rank, oder mit den nicht-analytischen Erfindern des Narzissmus, Paul Näcke und Henry Ellis . Freud erinnert uns daran, dass die beiden Erfindern des Begriffs ihn benutzt haben, um auf das Verhältnis von Menschen zum eigenen Körper hinzuweisen, welche mit ihm in einer Weise umgehen, wie andere Menschen mit dem Körper ihres sexuellen Partners verkehren.
In Anlehnung an die Sage von Narcissus, der sich in sein Spiegelbild verliebt, als Strafe dafür, dass er Echo verschmähte, ist Narzissmus mit der Liebe, die man dem Bild von sich selbst entgegenbringt, zu verstehen.
