Description: This excellent and comprehensive new book covers the rapidly growing use of deep brain stimulation (DBS) for the treatment of neurological and psychiatric disorders. Although largely experimental for use in psychiatric disorders, deep brain stimulation is common in the treatment of Parkinson disease and essential tremor, and within the next several years, this treatment modality will have more widespread indications in neuropsychiatric conditions. Written and edited by a group of clinician-researchers in the field, this book will be extremely useful for clinicians wishing to learn about this important new tool.
Purpose: The purpose is to "gather a group of experienced and recognized authors to review the current state of DBS in neurologic and psychiatric disorders." In addition, the editors wanted the authors to provide "a conservative and level-headed approach to the use of DBS with emphasis on objective assessment of clinical outcomes, particularly in the new and emerging applications." Indeed, the editors and chapter authors take a thoughtful approach to this emerging treatment technology.
Audience: The intended audience includes neurosurgeons, neurologists, psychiatrists, neuropsychiatrists/behavioral neurologists, and neuropsychologists. Any clinician or researcher in this field would benefit from reading this valuable book.
Features: Part 1 is an informative overview of the field that includes such topics as neuroanatomy, neurophysiology, the historical development of the use of electricity on the brain (although neglecting to mention electroconvulsive therapy), patient selection, mechanisms of DBS, and functional imaging in DBS. Part 2 reviews the use of DBS in movement disorders with interesting chapters on Parkinson disease and essential tremor, regarding stimulation of the thalamus, globus pallidus, subthalamic nucleus, the use of DBS in atypical parkinsonism, dystonia, and Tourette syndrome. Postoperative monitoring is covered in part 3, with important chapters on programming, neuropsychological issues, safety, fault testing, quality of life, and cost effectiveness. The last section covers psychiatric disorders and, at this point, more experimental indications of DBS in depression, obsessive-compulsive disorder, intractable cluster headache, epilepsy, and an interesting chapter on the future of the field. This is a scholarly work and, as such, each chapter concludes with important and relevant citations of the scientific literature. The index is quite helpful.
Assessment: This is a superb book covering the emerging treatment technology of DBS. Any clinician who wants to keep abreast of this important treatment modality, one that I believe will be more commonly used, should read this book. I highly recommend it.
