Purpose: The revolutionary advances made in molecular biology during the past quarter century are having a major impact on all areas of the practice of surgical pathology, including of the thyroid. A variety of novel genetic alterations responsible for familial and sporadic forms of thyroid cancer have emerged, resulting in the introduction of molecular tests for diagnosis. In addition, the conventional pathologic criteria for many thyroid lesions have been refined and new immunohistochemical markers have emerged. This book not only addresses the recent advances in thyroid pathology, but also elegantly describes the more general and practical aspects of both neoplastic and non-neoplastic thyroid diseases.
Audience: Although primarily targeted at practicing surgical pathologists, fellows, and pathology residents, this book has sufficient breadth that it can be a useful for radiologists, surgeons with a particular interest in thyroid surgery, endocrinologists, and researchers of thyroid diseases. The authors are renowned in the field and have used their wealth of experience to create a comprehensive and intelligible book.
Features: Initial chapters cover the basics of the embryology, anatomy, and physiology of normal thyroid, followed by a few chapters on non-neoplastic thyroid disease. The remaining two-thirds of the book is dedicated to the tumors of the thyroid. The final two short chapters describe the gross examination of the thyroid and the principles of molecular diagnostics in thyroid samples. All major chapters follow a relatively uniform format of definition, epidemiology, etiology, clinical presentation and imaging, gross and microscopic description, cytology, and differential diagnosis and conclude with special diagnostic techniques that include immunohistochemistry and a separate section on molecular genetics. One of the most impressive features of this book is the extensive use of high quality illustrations that include tables, charts, flow diagrams, routine histology images (hematoxylin and eosin, special stains, and immunohistochemistry), images of cytological smears, and electron photomicrographs. The use of tables and charts in the differential diagnosis sections to simplify and clarify the diagnostic criteria, particularly of the more diagnostically challenging lesions such as follicular neoplasms, makes this book quite useful, especially in routine practice. The information is up to date and easy to assimilate and the generous use of quality images greatly enhances the learning experience. Although the title includes molecular genetics of the thyroid, and no doubt it covers that aspect quite comprehensively, the core of the book is still based on conventional histopathology. By combining diagnostic surgical pathology, cytopathology, immunohistochemistry, and molecular genetics, the book effectively mimics the practice of contemporary surgical pathologists.
Assessment: This is a thorough and fairly wide-ranging book on a relatively small yet significant area of surgical pathology, authored by reputable pathologists who have a deep understanding of thyroid pathology in general and recent advances in this field in particular. The authors take a practical approach, indicating when multidisciplinary collaboration is required to achieve the most appropriate diagnosis. This book is also to be lauded for its portability and accessibility, and it certainly deserves a place on the library shelves of academic institutions as well as pathology trainees and practicing surgical pathologists.
