This is the third edition of a compilation of pharmacokinetic tables and primary journal references detailing the effects of renal insufficiency on therapeutic drug disposition. This book is intended to enhance the availability of information related to drug dosing in the setting of renal insufficiency. There is a general need to increase awareness of the necessity of individualizing drug dosing in special patient populations as well as to provide ready references and tools to aid prescribing and monitoring. These objectives are partially met by this book. The book is meant to be a pocket reference guide for physicians, pharmacists, and students. It would be of greatest value to geriatricians, nephrologists, acute care, and critical care physicians as well as therapeutic drug monitoring teams working in these areas. The authors are well-respected senior clinicians with considerable experience in the areas of nephrology and clinical pharmacology. With the exception of the cover photograph, there are no illustrations, a serious shortcoming. The references are for the most part recent and from good quality primary journals and monographs. The introduction to each chapter would be better if citations to important references were made. Also, the grouping of references by therapeutic area is somewhat of a hindrance save for the fact that the index lists both the chapter page as well as the bibliography page for each index item. Indicating the bibliography items in the index with bold typeface would enhance the usefulness of the index. Also, the tables would be improved by including references within the tables. The printing, layout, and formatting are of good quality. The authors, for the mostpart, have selected appropriate citations. They would have done better in some areas to choose well-referenced review articles rather than primary case reports (anecdotes). The introductory chapter on pharmacokinetics would be much more useful with the inclusion of illustrations, some practical graphic tools and nomograms for dosage adjustment, and mention of other important sources of drug information (electronic and printed) such as Hansten's Drug Interactions handbook. Also, an index of trade and generic names is missing. Given the pace of new drug development, this edition will be outdated within a year or two; the publisher would do well to make new editions available on an ongoing basis.
