Description: This is an augmented surgical atlas for core techniques in modern functional endoscopic sinus surgery (FESS). It is more than just a "how to," because it emphasizes building the surgeon's 3-dimensional conceptualization of an anatomically based surgical dissection. The first edition was awarded Best ENT Book of 2005 by the British Medical Association.
Purpose: The unique purpose of this book is to promote application of 2-dimensional endoscopic views to surgery in potentially complex 3-dimensional anatomic spaces. As alluded to in the preface, this is a necessary objective as endoscopic techniques continue to be extended beyond the paranasal sinuses. Systematic approaches are indeed necessary to apply minimally invasive techniques to the infratemporal fossa, parapharyngeal space, and intracranial structures.
Audience: The book would be appropriate for endoscopic surgeons-in-training, as well as accomplished FESS surgeons who seek to broaden their skills to include skull base techniques and to manage conditions beyond chronic rhinosinusitis. The author is one of the world's foremost experts in both of these pursuits.
Features: Initial chapters share critical details regarding the salient features of presurgical planning, such as room set-up and preparing the surgical field. This is followed by chapters organized by anatomic site and surgical technique. Dr. Wormald has an innovative method of using block modeling to build the surgeon's mental 3-D perspective, and this is clearly correlated with real endoscopic anatomy as well as CT in all three axes. The illustrations (in the text and on the DVD) are of remarkable quality, and each concisely relates the point at hand. As acknowledged by the author, the book avoids in-depth discussion of pathophysiology, although that may have been helpful at various junctures to further explain the rationale for the chosen technique.
Assessment: This atlas is the gold standard for the current state of endoscopic sinonasal techniques. Increased attention to endoscopic techniques beyond the paranasal sinuses, as well as additional videos and illustrations, help to maximize the proliferation of minimally invasive techniques so they may be applied to complex clinical scenarios.
