Description: This handbook not only comprises a multidisciplinary approach to diabetes management, but also addresses diabetes as a public health problem with solutions for prevention and treatments sensitive to diverse community needs. The sections on nutrition and self-management have convenient patient education material inserted in evidence-based provider guidelines; the figures and tables throughout concisely summarize chapter contents.
Purpose: The purpose is to provide persons involved in public policy, community health, clinical management, and ancillary healthcare with a comprehensive, evidence-based, community driven reference on diabetes in all its complexity. This book goes beyond the chronicity and complications for persons with diabetes to include prevention, self-management, cultural nuances, and special cases. It also reflects the effect diabetes has on the community, the healthcare system, and access to care. The book meets the authors' objectives.
Audience: The target audience is rather broad and includes medical practitioners, diabetes educators, public health nurses, and policy makers in finance and insurance. The organization of the book makes the content pertinent to each audience easy to find. The contributors are experts in their field and the editors have extensive background in the public health arena.
Features: The book presents diabetes as a community and public health problem of epidemic proportions. It explains the complexity of the disease process; the multifaceted treatment including diet, exercise, medicine, and self-management; and the effect culture, beliefs, and readiness for change have on patient outcomes. In addition, there is discussion of the direct and indirect cost diabetes has upon the community. The tables, figures, and patient education material is noteworthy. The section on medical therapies is brief, more of a listing than in-depth coverage, which does not conform to the detail provided in the rest of the book.
Assessment: This book is unique in its approach to the problem of diabetes. It views diabetes from a community or public health perspective. Its sections on cost, both direct and indirect, cultural diversity, prevention, and raising public awareness broadens the clinician's knowledge base. It summarizes effectively the pathophysiology, nutritional, exercise, and self-management aspects of diabetes and diabetes management.

Meet the Author

Donna Zazworsky, MS, RN, CCM, FAAN, is an internationally known expert in case management and disease management with community populations. She is managing partner for Case Manager Solutions, LLC, and director of Grants, Partnerships and Policy for St. Elizabeth of Hungary Community Health Center—-a large clinic that serves more than 18,000 uninsured in Tucson and the surrounding communities. On a personal note, Donna’s mother and two maternal aunts have experienced the devastating complications of Type 2 Diabetes. Six years ago, her mother suffered a stroke that has left her with left sided weakness. Both aunts died of diabetes complications related to above the knee bilateral amputations and end stage renal disease. Not only is diabetes prevention a personal motivation for Donna, but case management and disease management systems provide a primary focus for her work in understanding and reducing barriers for people with diabetes.

Jane Nelson Bolin, RN, JD, PhD, is a professor and researcher in rural health and disease management at Texas A&M. On a personal note, Jane has had gestational diabetes during both of her pregnancies. Knowing that the incidence of getting Type 2 diabetes at a later age is more than 70%, Jane practices a personal prevention program that includes diet and exercise.

Vicki Gaubeca, MPH, is assistant director of Public Affairs and Community Outreach for the Mel and Enid Zuckerman Arizona College of Public Health. On a personal note, Vicki has had Type 1 Diabetes since she was a teenager. She knows the day-to-day struggles of self management and hopes that this handbook will lend insight to the public health workers who help people with diabetes.
