Description: This is the 19th edition of the definitive book in the impossibly broad field of internal medicine. The previous edition was published in 2011.
Purpose: Harrison's aims to be a comprehensive book on the diagnosis, physiology, and treatment of common and uncommon disorders. It also provides commentary on the practice of medicine and general approaches to patients with symptoms of unknown etiology. While it does not claim to be entirely up to date, it still manages to account for developments of the last 12 months.
Audience: This is intended as a go-to reference for anyone in the practice of medicine, both generalists and specialists, ranging from medical students to seasoned practitioners, although it can be read by anyone with undergraduate-level education who has an interest in the field. In print for over half a century, it is the credible authority among books in internal medicine.
Features: With two volumes, the book is easy to navigate. The first volume approaches the patient by manifestations of disease (symptoms and signs), and the second, much larger, volume addresses disease by organ system. The index is easy to use. Despite the overlap of many diseases in multiple organ systems, I found most of the disease processes were addressed in only 2 to 3 separate areas of the book, preventing the need to frequently flip between chapters to cover all the material relevant to a topic. This edition includes a DVD with supplementary information and, while it has some useful videos, it is difficult to use practically. Making the online material available to purchasers of the book would be preferable, but I understand the economic reasons for making the online version a separate product. In any event, the publishers should take into account that many laptops do not even have a CD-ROM player anymore and this supplement might soon be obsolete. The content speaks for itself, it is always excellent and at the appropriate depth to make it possible read about a given narrow topic for 30-60 minutes.
Assessment: This remains the only must-have, comprehensive book in internal medicine. Frequent updates are justified, given the rapid evolution of the field. Overall, I think publishers have already made strides in planning for the eventual decline of large textbooks as the go-to references for the next generation of physicians. The book could more easily be a living text in an online/app format, allowing for real-time updates as guidelines change and trials are published. This will probably be the last Harrison's I own in print.
