Description:This is the long anticipated fourth edition of this gold standard textbook of neuropsychiatry. Although still entitled Lishman's Organic Psychiatry, Anthony David is now the editor and the chapters are authored by multiple contributors. The entire book has been revised with expanded and updated sections. This book was pivotal in expanding the focus of modern psychiatry to involve the psychopathology of brain disorders and remains THE textbook of neuropsychiatry.
Purpose:The aim of this edition remains the same as the first edition: to focus on the borderland between psychiatry and neurology. Neurology, although focused on the brain, tends to ignore psychopathology, while psychiatry "deals essentially with mental disorder, yet has had little in relative terms to do with the hardware upon which mind depends." This book has always attempted a comprehensive review of the psychopathology and cognitive deficits associated with brain disorders. The fourth edition lives up to this tradition with a comprehensive and thorough update.
Audience:The primary audience is psychiatrists. Neuropsychiatrists, behavioral neurologists, and any clinicians involved in neuroscience should use this classic as a reference.
Features:As in previous editions, part 1 focuses on principles and includes chapters on basic concepts of neuropsychiatry, the relationship between neuropsychology and psychiatry, and clinical assessment. Part 2 covers the major clinical areas: head injury, brain tumors, epilepsy, intracranial infections, cerebrovascular disorders, the dementias, endocrine and metabolic disorders, addictive and toxic disorders, movement disorders, and sleep disorders, with a section on miscellaneous disorders including multiple sclerosis, ALS, myotonic dystrophy, normal pressure hydrocephalus, and metachromatic leukodystrophy. All chapters detail the neurology as well as the psychopathology with rich descriptions. Chapters are referenced with relevant and timely citations of the scientific literature. The index is helpful, particularly with a large textbook such as this. The only criticism that I have is the retention of the term "organic" as the editor freely admits that the "line of demarcation is not hard and fast." I don't understand the justification for the statement: "while neuropsychiatry is the embodiment of the integrated approach to mind and body, it is not an excuse for woolly thinking or ignoring the very real distinctions between classes of disorders." This distinction is arbitrary at best. Is there an inorganic psychiatric disorder?
Assessment:Nonetheless, this remains the benchmark for all the other neuropsychiatry textbooks on the market and should be required reading for any card-carrying neuropsychiatrists (and psychiatrists) or behavioral neurologists!
