Description: This book addresses a commonly used osteopathic manipulative technique, the high velocity low amplitude technique, and the literature concerning it. The authors describe very well the uses and limitations of the technique, the need for additional research, and specifically what research should be embarked upon. The overwhelming value of this fourth edition is the CD containing videos of the proper applications of the technique.
Purpose: The purpose is to present a comprehensive treatise on the technique, the basis for its use, the research needed in the future, and a careful consideration of the contraindications, complications, and causes for failure.
Audience: The audience is clearly osteopathic clinicians, but the book also has great utility for those interested in physical medicine or alternative medicine.
Features: This book highlights the evidence-based literature on high velocity low amplitude manipulation. To their credit, the authors have succeeded, not that there is much literature specifically on outcomes, but they do correlate potential complications associated with similar activities and, where there is inconsistent literature, they cite and reference that literature. The figures suffer from single photographs attempting to show motion, but the authors have added annotations, e.g. stabilization points and plane of force arrows. The tremendously illustrative CD has videos that clearly demonstrate the techniques. In fact, this video is the best educational tutor I have yet to watch.
Assessment: This is the authoritative book on high velocity low amplitude technique, its successes, contraindications, complications, and causes for failure. It is an essential addition to the library of clinicians including this technique in their practice.

The conservative management of spinal pain syndromes requires the development of many skills including the necessary psychomotor skills to perform joint manipulation. Skill acquisition is not automatic and requires progressive steps of motor learning and frequent refreshment of learning. Peter Gibbons and Philip Tehan have taken the extremely difficult subject of the actual skill teaching in HVLA technique and produced what should prove to be a real milestone.
