This book provides the state-of-the-art developments in the field of "minimally invasive" or "minimal access" cardiac surgery. Organized into three major sections, the first section, Physiology of Injury, consists of a basic science review of the pathophysiological mechanisms underlying cardiopulmonary bypass and endothelial injury. The second section, Less Invasive Approaches to Coronary Bypass Grafting, is the most extensive section in the book, and has contributions from the pioneers of the various surgical techniques including videoscopic and thoracoscopic approaches. The final section addresses growing areas of less invasive cardiac surgery including valvular and congenital heart operation.

This book describes the new developments in the field of minimally invasive cardiac surgery (MICS). It is separated into three sections. The first part, "Physiology of Injury," consists of a basic science review of the pathophysiology of cardiopulmonary bypass and vascular injury. The next two sections deal with less invasive approaches to coronary artery bypass grafting and non-coronary MICS. The purpose is to provide the reader with an up-to-date review of MICS and to outline an optimal approach for this rapidly developing new field. The book is both a timely and extremely usable review of this area, and meets the editors' objectives. It is written primarily for practicing cardiac surgeons, but is also an excellent review for surgical residents. All the chapters are written by leading practitioners and active investigators in the field who are recognized authorities. The strength of this book is the excellent quality of the contributors. It is also unique in that it not only covers present minimally invasive techniques, but provides the physiologic justification for their development, with excellent chapters on the pathophysiology of cardiopulmonary bypass and endothelial injury. There is also an excellent chapter comparing minimally invasive coronary artery bypass grafting with percutaneous interventions, providing the reader with a perspective of where these new procedures fit into the overall spectrum of care. There are also chapters looking into the future, particularly into alternative approaches to vascular anastomoses and device-supported myocardial revascularization. The only shortcoming of this book is the generally poor quality of illustrations. This is a superbcontribution to the literature and is perhaps the most concise and comprehensive review of the field of MICS to date. It is must reading for surgeons interested in this field.

Description: This second edition begins with an overview of the pathophysiology of cardiopulmonary bypass and the endothelial injury that occurs during minimally invasive coronary artery bypass surgery. The remainder covers all aspects of minimally invasive bypass grafting, valvular, congenital, pericardial, arrhythmia, and robotic surgery. Every conceivable aspect of minimally invasive surgery, including quality of life and its economic impact, is included. The book pays particular attention to the available different technologies by devoting individual chapters to different companies and their advances. The previous edition was published in 1999.
Purpose: This new edition serves to update the reader in an area of rapidly growing technology. The authors wish to clarify and discuss the trade-offs between incision size and location ("minimally invasive"), cardiopulmonary bypass time, and expensive new technologies. The book sufficiently meets these worthy objectives.
Audience: The book could be beneficial for medical students, residents, surgical assistants, and practicing cardiothoracic surgeons. The authors are established authorities in the field.
Features: All areas of clinically relevant minimally invasive techniques in cardiac surgery are thoroughly covered. New technologies that may still be considered experimental by some are sensibly and honestly reviewed here. The photographs of emerging technologies are extremely helpful to the reader, but many would be clearer and significantly improved by presenting them in color.
Assessment: This is a very important addition to the literature because it presents an update of a rapidly changing field. New technologies are covered extensively. This book stimulates the always-learning surgeon to become interested and involved in the cutting edge of cardiac surgery.
