Description: This book provides the reader with up-to-date NANDA classifications linked to activities for the purpose of devising nursing care plans. This pocket guide can be used by nurses in the acute care or community setting. The previous edition was published in 2000.
Purpose: The purpose, according to the authors, is to aid practitioners and students in identifying interventions commonly associated with specific nursing diagnoses. The book links the NANDA taxonomy with interventions. NIC and NOC labels are also included. These worthy objectives are met.
Audience: The audience is defined as practicing nurses and students. The authors are well known and respected in their field.
Features: The book starts out with a definition and review of the nursing process and its application to practice. The third chapter uses sample assessments as methods of illustrating the organization of the nursing process and how NANDA applies, how to plan care, and finally how to implement and evaluate care. The document via SOAP or SOAPIER is contrasted with the more nursing-focused method called Focus Charting. The final sections of the book address the new taxonomy of NANDA diagnoses, associated health problems, and the Proposed ICD-10 version of NANDA. The latter is important for practitioners who need to use uniform terminology for reimbursement.
Assessment: Although there are many nursing care plan and nursing process books, this one's format as a pocket guide does not have direct competition. The book is compact enough to be taken in a lab coat pocket or at least carried by students to the clinical area.

This sixth edition is designed to aid the practitioner and student nurse in identifying interventions associated with specific nursing diagnoses as proposed by the North American Nursing Diagnosis Association. The book covers the nursing process; assessment tools; plans and documentation; patient situations; diagnoses and associated interventions; and health problems with related diagnoses. It is intended to be a guide for the practitioner and the student nurse. Practicing nurses and student nurses working in all clinical settings are the audience. References are current and include research studies. The book is well organized with subheadings for easy location of contents. It is pocket-sized and adequately spaced for easy reading. It is sturdy, attractive, and moisture and soil resistant. It provides the novice nurse with health problems along with the associated nursing diagnoses. The depth and scope of material provided is excellent. Assessment tools provide the reader with complete guidelines for data collection. Nursing diagnoses are organized according to nursing focus, allowing for accurate interpretation of diagnosis and for learning by the novice clinician. The section that contains rationale for care lacks depth. The title implies that rationale will be given throughout the text. The reader is not informed why fever is a sign of infection (endogenous pyrogen act on the temperature-regulating center in the hypothalamus) or why chills occur (the hypothalamus then activates the autonomic nervous system to stimulate increased muscle tone, and decreased perspiration and blood flow to periphery). I realize that space is a problem in providing in-depth physiologicrationale(s) for care, but the title leads one to expect that rationale for care is a major focus of the book.

    Clear and concise

    A must have for every student nurse. Nursing diagnoses, actions and rationales are clearly explained and cross referenced. A must when writing care plans.

    Student Nurses Pay Attention

    If you're a student nurse then this is the book for you. Our class bought a $50 care plan book and this is ten times better than that one. I highly recommend this book to any student nurse
