This handbook contains a system-organized selection of specific medical or surgical diagnosis-related assessment and care plan illustrations. Assessment is based on the authors' organization of nursing diagnoses as diagnostic divisions. Assessment focuses on nursing diagnosis as problems or needs of the patient, analysis aims for priorities, interventions are independent and collaborative with rationale from multiple disciplines, and desired outcomes/evaluation criteria are stated for each nursing diagnosis in broad terms. ""Clinical Pathways"" are illustrated for some nursing diagnoses and some medical diagnoses. The aim is to give definition and direction to development and use of individualized nursing care plans in multidisciplinary settings of medical/surgical hospital or community care. They express concern for the nurse in a changing healthcare system but propose help in reacting to change not leading as professional nurses with a knowledge base. The authors report designing the book for use in acute medical/surgical settings and community settings of posthospital care. The authors illustrate assessment, care plans, and some clinical pathways for selected medical diagnoses with possible associated nursing diagnoses. They deliberately avoid detail in interventions for the stated purpose of avoiding a procedure book format to allow individualization. They include a disk for electronic care plan making. This book will not meet the needs of the student or practicing nurse for guidance in developing individualized care plans. Efforts to provide rationales from multiple disciplines results in generalized, overly basic content. Inaccuracies regarding pathophysiology to projectedoutcome seem to derive from combining a nursing diagnosis with a medical diagnosis, as exemplified in the use of impaired skin integrity, a nursing diagnosis, for a diabetic with a foot wound. Content also becomes confusing here because some interventions address medical treatment of diabetes. Lack of nursing focus with research-based specific interventions is surprising given information currently available. No nursing research or classic nursing diagnosis references are listed.
Doenges, a clinical specialist, Moorhouse, a nurse consultant, and Geissler, a contract practitioner nurse consultant, all working out of Colorado Springs, Colorado, designed this text for both the practicing nurse and nursing student as a reference tool to use in acute care, community and homecare settings. Following introductory chapters on current issues in healthcare and care plan construction, the book is organized by body systems, presenting plans of care including information to assist the nurse in providing holistic care. Description: More than a care plan book, this book offers the application of a nursing focus to the planning of healthcare. It stresses the nurse's role as an active coordinator and planner of patient care.
Purpose: The purpose is to provide a nursing assessment tool for nurses to use to plan care. Secondarily, it seeks to assist the reader in obtaining competency in assessment skills and application of pathophysiology as related to planning care and setting priorities. The book meets its objectives.
Audience: The audience includes students as well as practicing nurses. Students will use the book to formulate and individualize plans for their patients and families. Practicing nurses will use it as a reference to apply the nursing process.
Features: The first few chapters lay the groundwork for the issues and trends in nursing, application of the nursing process, and critical thinking. This eases students, in particular, into the meat of the book — the application of the nursing process and the planning of care. A systems approach is used, which is appropriate for the purposes of this book. Use of colored textboxes throughout highlight important points. Flow charts and decision trees help with teaching and learning. Inside the front cover is a "Key to Essential Terminology," an excellent feature to help students see the bigger picture of the nursing process. The accompanying CD-ROM for the various specialty areas of nursing adds to the value of this book. The book is clear in its intent to provide guidelines for the nursing process and not to teach procedures.
Assessment: This edition provides up-to-date information with more emphasis on standards of care and documentation. It also stresses the dynamic role that nurses play in planning care today. The inclusion of a true nursing-based assessment tool is a great feature of this edition. Financing of healthcare is addressed in the first chapter, and although it does not develop into a lot of detail, there is enough substance to at least allow students a glimpse of some of the issues that affect care plans today.
