    Good material but missing pages!!

    The content of the book is excellent. However, my copy was missing the first 2-3 pages of Chapter and 1 and part of the index in the front. I found that very annoying. However, not all copies are like that. An intern on the wards had a copy of the same edition and it had the pages that mine was missing. So beware.
