Iontophoresis increases the penetration of drugs into the skin by electric current. Dexamethasone has anti-inflammatory, analgesic, and anti pyretic activity. Dexamethasone is synthetic glucocorticoid, which is used topically, intravenously, orally and in eye drops. Dexamethasone 21-phosphate disodium is a salt form pro-drug which converts in dexamethasone rapidly in blood. Dexamethasone 21-phosphate disodium is used for the iontophoretic administration because it is water soluble. The aim of this project is to determine the dexamethasone 21-phosphate disodium pharmacokinetics in skin following iontophoresis at different current density. A Microdialysis is highly reproducible and semi invasive method that allows to determine the drug concentration in the skin. Recovery shows the relationship between concentration of analyte in tissue and those in dialysate samples. In-vivo recovery cannot be estimated directly, hence retrodialysis method was chosen for probe calibration. Training and skill is requiring for microdialysis but still it provides highly dynamic continuous sampling and monitoring for several hours. The dialysate sample volume is very low so needs very sensitive analytical method for drug analysis. To determine dexamethasone 21-phosphate disodium in dialysate samples High Performance Liquid Chromatography (HPLC) method has been used. In HPLC method we used 0.1 M phosphate buffer: acetonitrile (70:30) mobile phase, flow rate is 0.5 ml/min, detection wave length was 240 nm and C18 column. The retention time was 5.4--5.8 minutes. The calibration curves for microdialysis samples were linear in the range of 100--10,000 ng/ml, with a correlation coefficient larger than 0.99. The lower limit of quantification was 100 ng/mL. The rabbit was chosen for the in-vivo pharmacokinetic study of dexamethasone 21-phosphate disodium. Four female pathogen free New Zealand albino rabbits were used. Microdialysis probes were inserted using sterile needle specific for microdialysis. The retrodialysis was done by using 10000 ng/mL dexamethasone 21-phosphate disodium solutions as perfusate. The iontophoresis patch was wetted with 0.35 ml of a solution made with 0.5 mg dexamethasone 21-phosphate disodium in 1 mL water:glycerin (50:50). Current density of 100, 200 and 300 muA/cm2 was applied for 60 minutes according to a cross-over design. The results shows that increasing the current density increases the penetration of the drug in the skin, but the result in exposure is linear across the applied current density. The half life in skin is 27--30 minutes for the current density 100 and 200 muA/cm2 when current density is 300 muA/cm 2 the half life is about 77 minutes. And the results show linearity of current density with AUCall and Cmax. More studies need to be done to better understand the elimination phase of DSP in skin.
