Features: The pocket cards are divided into sections. The items are arranged in an order that mimics the patient evaluation process and that makes the list easily accessible. It includes identification data, chief complaint, review of systems, vital signs and tests, social history, substance abuse history, psychiatric history, mental status exam, physical exam and assessment (which includes DSM-IV TR coding) and finishes with treatment plan and recommendations.
