Ivan Ilych is a bourgeois from a good family, of fair intelligence, constantly worried about the opinion of other people. Even his marriage is flat: he married his wife more out of social convenience than love. Everything is going along fine, until the protagonist suffers a trivial domestic accident. Falling from a stool Ivan Ilych accuses severe pain, a pain that will become increasingly disabling as time goes on, forcing him to consult numerous doctors in an attempt to recover. The illness thus changes his priorities, he becomes impatient with the noisy indifference of healthy people, no one understands or listens to him, except for one muzik, a villager, who from the day of the accident takes care of him. Thus Ivan will be led to reexamine the scale of values that had inspired his life, in a painful journey toward the truth.
