Overview

The first edition of The Paris System for Reporting Urinary Cytology introduced a completely new paradigm for detecting bladder cancer by urine cytology. This system concentrated on defining morphological characteristics of the most clinically significant form of bladder cancer, High Grade Urothelial Carcinoma. This new approach has been widely accepted throughout the world, and has become part of the daily practice of cytology. Considering that the first edition of The Paris System (TPS) introduced a new model of urinary cytodiagnosis, verification and expansion of initial material and data were anticipated. Based on evolving knowledge and readership requests, the group of highly experienced authors have created a new edition of TPS. This second edition includes areas and issues not originally covered. A new chapter on urine cytology of the upper tract, a rarely addressed topic, has been introduced. Description: This is an overview of the standardized Paris System for reporting urinary cytology (developed in 2016) with updates from the first edition. The second edition adds data and analysis from prospective studies of relevant diagnoses based on the Paris System criteria; these data substantiate the reporting system as a useful tool in guiding management. The second edition also expands on many topics included in the first edition, as well as adds more illustrations and photographs as a complement to the descriptive text.
Purpose: The intent of this text is to provide an overview of the use of The Paris System (TPS) for Reporting Urinary Cytology primarily in the context of pathogenesis, diagnosis, and management of urothelial carcinoma.
Audience: This is a useful reference for pathology residents, cytopathologists, and uropathologists. Urologists interested in the finer points of urinary cytopathology may also find this book useful.
Features: This book comprises 12 chapters. The early chapters discuss pathogenesis of urothelial carcinoma and assessing urine specimen adequacy. The following chapters discuss the spectrum of possible findings in a urine sample, including negative findings, atypical urothelial cells, suspicious for high-grade urothelial carcinoma, and positive for high-grade urothelial carcinoma. The remaining chapters cover general cytopathology of the upper urinary tract, non-urothelial malignancies and miscellaneous lesions, relevant ancillary studies, cytopreparatory techniques, risk of high-grade malignancy, and clinical management of urothelial disease. The book ends with a history of urinary cytology.
Assessment: This is an excellent overview of TPS presented expertly in a relevant clinical and pathological context. The book is well organized, concise, and makes good use of figures and illustrations to demonstrate key points. The beginning of each chapter also includes a summary of changes in the second edition, which will help experienced readers easily identify what is new and areas they may wish to focus on for continued learning. The book also includes important criteria and cytomorphologic features of various diagnoses, presented in a convenient bullet-point style for easy reference. Subsections of each chapter contain helpful "Explanatory Notes," which are essentially diagnostic pearls listed separately from the main body of the text. I find these explanatory notes very helpful as they repeat points from the main text, reiterating take-home points from each subsection in an easily digestible fashion. Microscopic images in each chapter are high-quality. Overall, this is a great reference book to have for experienced uro- and cytopathologists, and a helpful guide to the use of this reporting system for beginners.
