In Chinese medicine, diagnostic information is gathered by the four examinations (si zhen). These are looking, listening-smelling, palpation, and questioning. Looking consists of looking at the patient's affect, their facial complexion, and their carriage and constitution, and their movement as well as visually inspecting the area of complaint. It also involves inspecting the tongue shape, color, moisture, and fur or coating. Listening-smelling refers to listening to the patient's tone and timbre of their voice as well as to the sound of their breathing and the sound of any coughing and wheezing. It also includes smelling of any bodily discharges (although nowadays this is mostly accomplished by questioning). Palpation includes the manual examination of any areas of pain or disease as well as the feeling of the arterial pulses at both wrists. Questioning refers to asking the patient about the onset, course, and symptoms of their present complaint as well as questions about all their bodily functions. Information gathered by these four methods is then categorized into general signs and symptoms, tongue signs, and pulse signs. It is the confluence of these three categories of information which then establishes a patient's Chinese medical pattern of disharmony.
