Description:This is the third edition of this very helpful compilation of forms and protocols to be used in a psychiatric practice. The first edition was developed by Richard Jed Wyatt, MD, who was an NIMH scientist and a practicing psychiatrist. With his passing, Dr. Robert Chew completed the revision of this very practical reference for the busy psychiatric practice. The book is a valuable contribution to clinical psychiatry.
Purpose:The purpose is to have a "convenient source for history and documentation forms, rating scales and instruments, and information handouts for patients and families about psychiatric disorders and medications." The authors have succeeded in producing a very relevant and practical set of tools for the clinical psychiatrist.
Audience:The intended audience is, of course, practicing psychiatrists. Trainees who are finishing their psychiatry residency programs should purchase this invaluable book for their psychiatric practice.
Features:The book contains 240 pages divided into three parts. Part 1 contains model forms for an office practice: initial letter to patients, self-assessment form for patients, authorizations for medical records, patient-physician agreements, a sample invoice form, and even a collection notice. In addition, this part contains essential rating forms for assessing patients such as the Hamilton Rating Scale for Depression, Yale-Brown Obsessive-Compulsive Scale, to name a few. Part 2 contains information handouts for patients and their families about common psychiatric disorders and includes advocacy and support group contact information. Part 3 is a set of handouts regarding medication information for patients and families including antipsychotics, antidepressants, mood stabilizers, antiparkinsonian agents, antianxiety/sedative-hypnotic agents, ADHD treatments, and cognitive enhancers. An invaluable bonus is that a CD-ROM of all the forms is included in a PDF format. The index is helpful and relevant references are listed at the back of the book.
Assessment:This edition is updated to the 2005 practice of psychiatry and continues to be an invaluable set of tools for the modern psychiatrist. No psychiatric practice should be without it!
