Overview

The term “dermatologic emergency” is rarely used, and a dermatologist is typically not the first physician called when a patient presents in the emergency department. However, largely under-recognized dermatoses can be life-threatening if not diagnosed and treated in a timely fashion. Buka's Emergencies in Dermatology addresses the need for a concise, portable reference that can be used by emergency medicine and dermatology physicians and residents alike.

This book, heavily illustrated with color images, proves to be especially critical if a community-based hospital does not have a dermatology department and the responsibility for initial diagnosis falls to the emergency room or critical care physician. However, since the majority of cutaneous outbreaks are not life-threatening, it is often the case that emergency department personnel are less equipped to diagnose these conditions correctly.

About the Author

Dr. Robert Buka is the Section Chief at Mt. Sinai School of Medicine and his particular expertise is in vascular lasers, skin cancer, and acne. He speaks at the annual meeting of the American Academy of Dermatology on topics ranging from complex eczema to techniques in dermatologic surgery. His articles have been published in dermatology and plastic surgery textbooks as well as a number of medical research journals.

Dr. Uliasz is a clinical instructor of dermatology at Mount Sinai and has published numerous articles and chapters in leading dermatology journals and textbooks, and regularly presents her research at national conferences throughout the country. Dr. Uliasz specializes in both medical and cosmetic dermatology with a special interest in acne, psoriasis, and skin cancer.

Dr. Karthik Krishnamurthy Director of Dermatology at Jacobi Medical Center, Director of the Cosmetic Dermatology Clinic at Montefiore Medical Center, and Assistant Professor at Albert Einstein College of Medicine, has a special interest in academic dermatology and research. Widely published and a nationally-invited speaker, he is also the Editor-in-Chief of the Journal of the American Osteopathic College of Dermatology.

Description: This book discusses an assortment of dermatological diseases that can have potentially life-threatening complications. Acute conditions arising from arthropod bites, surgical problems, and infections are also covered. Color photographs and tables summarize treatment options and compare clinical presentations of similarly appearing diseases.
Purpose: The authors' purpose is to provide a reference for emergency room physicians, dermatologists, and residents for recognizing presentations of dermatological conditions that can have serious morbidity and be potentially fatal. The book offers an outline for thinking about the differential diagnosis for serious skin diseases one would not want to miss. The chapters are organized according to etiology: bullous dermatoses, vasculitis, collagen vascular disease, drug reactions, etc. Readers must have some prior dermatology background to choose the correct chapter(s) to read in order to make appropriate decisions regarding patient management.
Audience: The audience is emergency room physicians, dermatologists, and residents.
Features: Variable in-depth discussions of etiology, presentation, epidemiology, laboratory evaluation, clinical complications, and treatment for each disease are presented. When faced with an entity that may be an emergency, it is helpful to have specific clinical descriptions of the skin lesions and the clues to pay attention to that would alert the clinician to differentiate a serious condition from a nonemergency condition. The color photographs are overall quite good quality. However, more photographs showing more detail would be helpful. Some of the photographs of oral lesions are blurry and it is difficult to see the lesions. The discussion of treatment options is detailed for pemphigus, but not as detailed for other entities. Tables summarizing treatment in detail for these emergency conditions would be helpful. Having to read through text makes the book less of a quick reference.
Assessment: The authors have compiled important dermatologic diseases that both dermatology residents and physicians who are not dermatologists need to be aware of. I look forward to the next edition where there are more detailed color photographs, hopefully for all the entities discussed, and a little more consistency in the presentation of the information. It is a useful outline for nondermatologists and medical students and residents seeking an introduction to potential life-threatening skin conditions.
