Admir Hadzic, MD, PhD (New York, NY) is Director of Regional Anesthesia in St. Luke’s-Roosevelt Hospital Center. He is also Professor of Anesthesiology at College of Physicians and Surgeons of Columbia University.

Description: This comprehensive book on regional anesthesia and acute pain management leaves nothing to the imagination. It covers every possible block and every published technique. The text is littered with pictures and images to the extent that the book has increased in size by a third. The previous edition was published a decade ago, and the authors have updated it by including more nuances.
Purpose: The book strives to be a gold standard in regional anesthesia. It aims to cover everything that relates to the practice of regional anesthesia. These are noble objectives, but this has also added to the size of the book, making it less portable.
Audience: The audience is any clinician who practices regional anesthesia, regardless of their training or background. The authors include a few from outside the U.S., giving the whole effort an international flavor.
Features: The almost 100 chapters are divided into 14 sections over 1,500 pages. The book starts with discussion of embryology, moves on to a detailed discussion of microscopic anatomy, followed by a review of local anesthetic pharmacology. Part 3 covers techniques of regional anesthesia from head to toe including neuraxial blocks, head and neck, and ophthalmic blocks using ultrasound guidance. The next four sections discuss regional anesthesia on the obstetric floor, in the emergency room, and for patients with different needs like the elderly, children, and patients with different diseases or opioid dependence. Complications from regional anesthesia and their evaluation, as well as management of LAST are reviewed separately. Outcome data, financial aspects of regional anesthesia, and how to teach this art is briefly touched upon in a few chapters. In the last part, the authors review landmark based techniques for common limb and truncal blocks using nerve stimulation. The best part of the book is the chapter on statistics. The author very successfully explains the nature of often-used statistical tests in research. The other main focus of the book is microscopic anatomy of nerves, and its different fascial coverage. The book has more pictures of microscopic anatomy of nerves than any other regional anesthesia book. Overall, the book does justice to the regional anesthesia part of the title but not to the acute pain management part. There is no discussion of how to manage chronic pain patients on the floor after surgery and no chapter on the experience of patients after major abdominal surgery with or without thoracic epidural. The sole focus of the book is on placing blocks, and what happens after 24 hours is not addressed. In that sense, it does not live up to the second part of the title.
Assessment: This is an excellent, comprehensive book on regional anesthesia. It covers anatomy in detail, which is a cornerstone of successful and safe regional anesthesia practice. The size of the book makes it hard to carry around. There is no accompanying online version and there are no videos of any procedures, two things that will make or break this book in the long run.
