Overview

Diabetes mellitus, particularly non-insulin-dependent diabetes Type 2, is a common disease and, even though insulin has been around for seventy years, this endocrine disorder still reduces the life expectancy of diabetic patients because of the development of long-term complications, including hypertension. Hypertension occurs twice as often in diabetic patients as in non-diabetic ones. The combination of diabetes and hypertension creates a greater risk of coronary artery disease which is the major cause of death in westernized societies. Bryan Williams and his international team of collaborators provide the reader with their views for treatment of diabetes associated with hypertension and the prevention of subsequent coronary artery disease and other disorders.

Description: This book provides a comprehensive overview and interplay of two common diseases, hypertension and diabetes. It summarizes up-to-date knowledge about the etiology of these two conditions, as well as the complications that arise from these diseases. It also addresses the most appropriate ways of treating complications associated with diabetes and hypertension.
Purpose: The authors' intention was to recognize diabetes mellitus as a cardiovascular disease by virtue of its complications. This may help in recognizing diabetes as a major risk factor for cardiovascular events. It summarizes the outcomes of all the major studies related to diabetes mellitus and hypertension that were conducted in the past few decades.
Audience: This book will be helpful to a wide audience, including medical students, residents, general practitioners, endocrine and kidney specialists, cardiologists, and all others who treat diabetes mellitus and hypertension.
Features: This book covers epidemiology, pathogenesis, and treatment of hypertension in diabetes mellitus, as well as common target organ complications of these two diseases. The impact of ambulatory blood pressure monitoring is well addressed. Chapters on retinopathy, RAS evaluation, mechanism of action of antihypertensive medications, and treatment of hypertension in pregnancy are particularly well summarized. There are a few shortcomings. The blood pressure target range is outdated in many of the chapters (it is up-to-date in chapters 12, 15, and 17). The importance of good blood sugar control in acute MI and post-CABG could include more studies in addition to DIGAMI. Diagnosis of microalbuminuria and assessment of kidney function is not well addressed (use of spot urine for microalbumin, measuring creatinine clearance, etc). It would be helpful to have an SI conversion table towards the back of the book. There is no consistency in writing type 1 and type 2 DM (type I, type II). Abbreviations in fig. 11-1 and table 1-2 need an explanation and the figure 12 on page 133 should be move to 143.
Assessment: The book is well formatted and easy to read. It summarizes all the crucial recent studies that have a major impact on our approach to treatment of hypertension in diabetes mellitus.
