Description: This handbook describes basic concepts in assessing, diagnosing, and treating common peripheral vestibular disorders and ruling out central vertigo.
Purpose: The purpose is to simplify the diagnosis and management of dizzy patients and even provide a guideline for general practitioners. Given the complexity and frustration that physicians have working with patients with this common complaint, this is a worthy objective. The book certainly does provide a large differential for patients with dizziness and, thus, a good starting point for the management of these patients. The book is intended to act as a catalyst by providing basic fodder as opposed to a comprehensive source.
Audience: The audience includes a wide variety of practitioners, from medical students to otolaryngologists and neurologists.
Features: The book covers a quick differential of central and vestibular causes of vertigo, and for those with peripheral disorders, it outlines treatments that include medical treatments and vestibular rehabilitation. One of the unique characteristics of this small book is the conversational tone. For example, the author advises encouraging some patients to accept their problem and live with their "vestibular inadequacy" and frankly "do not keep bringing them back." The book also mentions unique tests and drugs that I have not encountered during my training, such as the glycerol test for Meniere's disease, cinnarizine, and Cawthorne Cooksey exercises. The most thorough chapter is the one on vestibular rehabilitation exercises, with its many illustrations and detailed explanations. Among the shortcomings are typographical errors, including recommending "in vitro fertilization" under management of acute attack on page 98, when it is clear the author intended "intravenous fluids." Also, the organization of the book is a bit haphazard. Though on the surface the division of the subjects makes sense (causes of vertigo/clinical investigations for vertigo/treatment of vertigo), the points are so basic and brief as to convey the notion that the author is jumping around. It is difficult to get a comprehensive idea of how to treat specific disease processes. Perhaps the book would be more accessible if it was organized by disease process.
Assessment: This is a good starting point in that it provides a wide differential for patients with vertigo. It also includes a nice detailed chapter on vestibular rehabilitation and quick tips on treatment of vertigo. However, given the organization of the book, I would find it frustrating to use as a clinical guide.
