Dr Satish Mittal MBBS is a General Medical Practitioner and former General Practice Principal, East Molesey, East Surrey Health Authority, Surrey, UK; and Clinical Governance Lead and Chairman of Clinical Governance Committee, East Elmbridge Primary Care Group, Esher, Surrey, UK. He previously worked as a registrar in cardiology and general medicine. Description: The author attempts to review the literature concerning the metabolic syndrome and outline what primary care practitioners should know. This is a fairly comprehensive book highlighting the association with various common diagnoses encountered in practice such as obesity, dyslipidemia, insulin resistance, and diabetes. Following this discussion, the author summarizes the biochemical abnormalities found in muscle tissue and liver and the contrasting views of whether the underlying problem is a defect in glucose metabolism or in lipid metabolism. This book is organized in a question and answer format that adequately covers the material, but makes it difficult to use a reference.
Purpose: The author's stated purpose is to provide a practical approach to the understanding of insulin resistance, obesity, hypertriglyceridemia and the subsequent proinflammatory and prothrombic states. In light of the discussion of whether the metabolic syndrome is, indeed, a discrete entity, this is a worthy venture. The author has provided a comprehensive approach to answering this question.
Audience: This is a reference for medical students through attending physicians as well as anyone who wishes to study or obtain additional information on the metabolic syndrome. The book is written at an understandable level for all levels of scientific students of physiology.
Features: This is an analysis covering the genetics, leading to the basic physiology which becomes clinically apparent through specific diagnoses. The author has produced a comprehensive presentation of the entire spectrum. The somewhat outdated references are a shortcoming. One interesting aspect is the comparison of the ATP III guidelines; the World Health Organization criteria; the International Diabetes Federation Criteria; and other criteria proffered by various organizations. There is also a section on the use and comparison of the current medications in the treatment of dyslipidemia and diabetes associated with the metabolic syndrome. The index is complete and helpful.
Assessment: This book has a very limited scope, but the author presents a very comprehensive summary of the focus, the metabolic syndrome. Unfortunately, this is a subject about which there are many unanswered questions and the references are a bit dated. Nevertheless, for this point in time, the book is comprehensive and an important library addition.
