Overview

The literature on NEC (necrotizing Enterocolitis), especially in the major textbooks, merely recapitulated what had been written in the past; there has been no new guidance. NEC was viewed as a fait accompli; a sometimes-dire disease that was to be confronted only after it reared its perfidious head. This monograph asks the reader to rethink their approach the NEC. Although, there are no compel others to pursue a way to intervene before the die has been cast. Thus, this book is a promissory note and a challenge.

This is a small monograph on necrotizing enterocolitis (NEC). It consists of nine chapters on the major topics usually addressed in relation of NEC, specifically historical development, predisposing causes, immunology, radiographic findings, bacteriology, and the medical and surgical management of NEC. The editor states the purpose is to produce a "paradigm shift" in the way we currently think about and approach the treatment of NEC. Further, he states the text represents "a promissory note and a challenge," but there is real question whether this manuscript does either since it represents rather standard and well-known data concerning this vexing problem. The audience for this text is pediatricians, pediatric surgeons, and perhaps students in either discipline. Within the nine chapters is a quite comprehensive review of what is currently know about NEC. Dr. Schullinger nicely recounts the historical record of NEC recognition and the first major reports from the Hospital for Sick Babies in New York. Subsequent chapter contributors report on and summarize the information currently available on bacteriology and immunology. In the fourth chapter the discussion is on radiological investigation to confirm or follow suspected cases of NEC. Two chapters are devoted to medical and surgical therapy. The contributors cover nicely current therapeutic modalities and have good bibliographic lists at the conclusion of each chapter. Finally, the monograph concludes with a discussion of pathology, nutritional support of NEC babies or the consequent "short bowel syndrome" seen in so many of these children, and the possibility of small bowel transplantation in post-NEC short bowel syndrome. Thisis a nice, single source of currently available information on necrotizing enterocolitis. It is a rapid, in-depth review or introduction to the subject. However, it is a summary of information already available in other sources.

While not presenting firm conclusions, the nine papers collected here challenge the contemporary thinking on NEC, and demand a new approach to the treatment of the disease. They cover the history of necrotizing enterocolitis investigation, bacteriology, immunology, radiology, medical and ICU management, surgery, pathology, nutritional concerns, and the use of small bowel transplantation.
