Description: This is the second edition of a book posing questions and answers in a multiple-choice format designed to prepare readers for certification examinations in neurological critical care. The first edition was released in 2014.
Purpose: The purpose is to provide questions designed to provide a systematic review of medical and surgical illness associated with critical neurological conditions from specialists in the discipline.
Audience: Providers preparing for standard examinations and senior trainees seeking to identify learning points in neurological critical care are an appropriate audience for this book, written by a faculty of neurologists and neurointensivists from medical centers in the United States.
Features: The book is divided into two parts. The first chapters focus on classic neurological disease states with questions intended to review pathophysiology and therapy. For example, the book describes ischemic and hemorrhagic disorders, as well as infectious, endocrine, inflammatory and oncologic problems. Separate sets of questions describe management of the perioperative patient and pharmacology specific to neurocritical care. A second broad group of chapters include questions related to general critical care. For example, cardiovascular and pulmonary disorders are examined at length with smaller chapters describing renal disease, infectious problems, hematological issues, and gastrointestinal disorders, with a presentation on management of brain death. Later chapters provide a brief survey of trauma and burns, ethics, research principles, procedures, and monitoring. A brief but important concluding chapter presents clinical cases allowing readers to examine treatment options. Each chapter begins with a list of multiple-choice questions. Sometimes factual data is requested, while other questions pose clinical scenarios. The second portion of each chapter includes detailed answers to the questions along with references. For the most part, the references come from the primary literature and date to within one year of publication. Secondary sources tend to be older. Tables and line drawings are infrequent but reproduce well. The only images are occasional CT scans. The table of contents attributes authorship and lists chapter titles while a concluding subject index allows the reader to find both questions and associated answers with rationale material.
Assessment: This collection of over 700 questions provides a helpful survey of both neuroscience and general critical care core knowledge. The questions are readable, but the language is not as precise as on certification examinations. The rationales and references presented for each question are well prepared. Subsequent editions might be improved with better images of radiographs, EEGs, and illustrations of classic physical findings. Finally, some of the chapters discussing general critical care use dated secondary sources. Perhaps contributors in these disciplines could be employed.
