About the Author

Robert Wachter, MD is professor and Chair of the Department of Medicine at the University of California, San Francisco. He is generally considered to be the “Father of Patient Safety” and in 2015, Modern Healthcare named him the most influential physician-executive in the United States.

Kiran Gupta, MD, MPH is an Assistant Professor of Medicine at the University of California, San Francisco.

Description: This book comprehensively covers all types of medical errors, the basic principles of patient safety, quality, and value, as well as information technology. It also addresses workforce issues, accreditation and regulatory issues, risk tools, accountability, and how to build a safety program. The previous editions of this book were published in 2012 and 2008.
Purpose: The purpose is to enhance the knowledge of healthcare professionals on patient safety practices and issues. The authors carefully developed the objectives to reflect current practices in patient safety.
Audience: The authors suggest a wide-ranging audience of healthcare professionals and graduate students. The book is not suitable for undergraduate students as their knowledge level is not yet developed. Both authors are medical physicians with academic appointments. The lead author is a well-published, accomplished expert, while the coauthor has completed a fellowship in patient safety and received an MPH from the Harvard School of Public Health.
Features: This is a comprehensive book on patient safety. Throughout, it uses a variety of educational methods to meet the objectives. It defines, clarifies, and provides processes for conducting a root cause analysis. In addition, it shares expert guidelines for a revised root cause analysis process.
