About the Author

Prof. Antonio M. Esquinas, MD, PhD, FCCP, FNIV, International fellow of the American Association for Respiratory Care (AARC), is a critical care specialist and staff physician at the Intensive Care Unit at the Hospital Morales Meseguer, Murcia, Spain. He is the Director of the International School of Non-Invasive Mechanical Ventilation, International College of Experts in Non Invasive Ventilation, Director, Academy of Non Invasive Ventilation and Director International Non-invasive Ventilation Fellowship Program and President of International Association of Non Invasive Ventilation. His main research activities are related to noninvasive mechanical ventilation in pulmonary and critical care. Prof. Esquinas is the author of more than 670 scientific publications (402 on peer-reviewed journals), editor of 60 books (published in English, Portuguese, Spanish and Chinese) and author of 325 chapters of book/position paper. He was invited speaker at 356 presentations and presented 450 oralcommunications or posters at national and international congresses. He acts in international networking groups promoting education programs and research in noninvasive ventilation.

Description: This book describes the engineering, physiology, and applications of humidification for gases used as part of respiratory therapy in the intensive care unit. This second edition is an update to the original version, which was published in 2012.
Purpose: The purpose of the book is to present clinical indications, appropriate and necessary monitoring, standard evaluation of results, and complications of humidification. Humidification practice may have a significant impact on the realization of clinical objectives related to respiratory support in critical care.
Audience: Intensivists, senior trainees, and respiratory therapy personnel will derive great benefit from the book. This work originated from several major European medical centers.
Features: The book includes 37 chapters divided into nine parts. Initial chapters describe the rationale, importance, and basic technology involved in providing heated humidified gas to critical care patients. The book also presents technologies including types of humidifiers, heating wires and other heat outlets, moisture exchangers, and measurement techniques for hygrometry of inhaled gas during mechanical ventilation. The literature review includes studies demonstrating the adverse effects of inadequate humidification, such as mucosal injury and airway collapse and disruption, along with ineffective sharing of oxygen with the patient and clearance of carbon dioxide secondary to alveolar injury associated with exposure to dry and cold gas provided as part of respiratory support. Humidification is titrated to avoid these problems. Following humidification therapy, the book discusses applications of humidification strategies for invasive and noninvasive mechanical ventilation, with particular emphasis on high-flow oxygen therapy. Subsequent chapters describe applications of humidification in patients with complications during mechanical ventilation, including extubation failure, exacerbations of chronic obstructive pulmonary disease (COPD) and asthma, as well as patients with both acute and chronic neuromuscular conditions. Patients requiring tracheostomy present special challenges for humidification and gas exchange, as the important resources of the upper airway are bypassed. Several chapters address these concerns, particularly the need for adequate secretion management. A related challenge in respiratory support is the treatment of secretions and gas humidification with special modes of ventilation such as high-frequency percussive ventilation. The book discusses strategies that complement humidified gas administration such as chest physiotherapy, chest wall oscillation, and oscillatory positive inspiratory pressure, along with cough augmentation and medications used to facilitate secretion removal. The chapters are concise and frequently introduce vocabulary specific to the topic presented and include reference lists of varying lengths dated within 3-10 years of the book's publication. Unfortunately, tables of data and diagrams are relatively infrequent. The table of contents lists chapters, attributes authorship, and provides subheadings. A brief index concludes.
Assessment: This attractive book describes a less appreciated, but vital component of ongoing respiratory therapy. Because this is a relatively unfamiliar topic to many intensivists, additional definitions and illustrative tables and figures would be helpful. Fortunately, this edition includes discussion relevant to newer modes of ventilation. To achieve the greatest benefit from this book, complementary reading may be necessary.
