Description: This book builds on an already excellent first edition by adding the most recent basic information and cutting-edge surgical improvements to the management of distal radius and ulnar fractures.
Purpose: The purpose is to provide updated information in these areas. These worthy objectives are met in most instances.
Audience: The book is written mainly for hand and general orthopedic surgeons and traumatologists. Orthopedic trainees and therapists represent another target audience. The authors, Diego L. Fernandez, MD, University of Bern and Lindenhof Hospital, Bern, Switzerland, and Jesse B. Jupiter, MD, Harvard Medical School and Massachusetts General Hospital, are internationally recognized leaders on injuries of the wrist. They are most credible and qualified to write on this subject.
Features: The authors have used the same format as the original excellent work with some important changes. Two new contributing authors have expanded on the radiocarpal fracture - dislocation and malunion of the distal end of the radius chapters. Also, a new and important chapter on acute and chronic derangement of the distal radioulnar joint associated with distal radius fractures is added to the contents. The chapter on surgical techniques discusses new internal fixation devices such as the pi plate system and the trimed plates. The more recent system of plate and screw-in metal pegs is not discussed. Bone graft substitute such as Norion SRS and the osteoinductive products (eg OP-1, NOVOS) are discussed as ways to enhance bone healing in comminuted fractures. Unlike the original book, there is less repetition of the authors classification system. Chapter one on the historical perspective is the same as the first edition. Epidemiology, mechanism, functional and radiographic anatomy are all very well covered in the early chapters. The authors make excellent use of numerous photographs and illustrations. The references are current and extensive.
Assessment: This is perhaps the most important contemporary work on injuries of the distal forearm. We are witnessing a renaissance in the management of wrist fractures. The old eponyms of Colles, Smith and Barton are replaced by detailed description of fracture types based for the most part on mechanism of injury. The early reliance of the external fixator as the prime stabilization device is being augmented or replaced with wire or plate fixation techniques. The active senior citizen needs better care of his or her wrist fracture. The authors have recognized this need and they have presented an excellent management plan. I very highly recommend this textbook to all orthopedic surgeons and traumatologists caring for adult patients with distal forearm fractures.

This book first describes in great detail the subtle differences between the various types of distal radius fractures and then stresses a specific management plan. Although not clearly articulated in the prefaces, the authors' objectives appears to be twofold. First, they attempt to combine their extensive personal experiences of two orthopedic surgeons in dealing with wrist injuries. Second, they amass the world's literature past and present on the topic, with particular reference to fracture classification, and they present in a logical manner the preferred treatment plan. Their objectives were fully met. The text is primarily written for orthopedists and traumatologists caring for patients with fractures. It is expected to become the primary source for orthopedic residents on this subject. Both authors are internationally known for their authoritative knowledge on distal radius injuries. The book is divided into 11 chapters. The topics include a historical perspective, epidemiology, mechanism, classification management of specific fracture types, complications, and malunion. The references are extensive and current. The quality of illustrations and photos are first rate. The repeated use of the classification illustration may be somewhat overdone. The authors have presented convincing arguments to the reader to do the following: (1) stop using eponyms such as Colles, Smith and Barton and (2) consider transferring the management of more involved fractures from the emergency room to the operating room. The book is strongly recommended for purchase by all surgeons caring for these injuries and orthopedic residents. It is also recommended for all medical libraries. The authorshave done a brilliant job and they must be congratulated.
