In the cruel and oppressive land of Eshar, fifteen-year-old Stephen Laing receives his father's will years after his death, leading him to a series of journal entries that were written by his father about the world around him. He discovers that his father left this journal to inform him and his ten-year-old sister, Faith, about Eshar, their home, and the possibility of escaping it. The two learn that beyond Eshar's harsh society, there is a place free of sadness, death, and pain, a place where you can live freely without having to act, think, and live how the government wants you to. This place is known only as the world beyond reality, and Stephen and Faith are willing to give up everything to get there. The more they learn, the more they begin to doubt and distrust the world around them. Stephen, at his core, wants to know the truth. But there is a cruel power that stands in his way, and it will do anything to prevent him from uncovering it. That cruel power has a name: the Ministry of Information. They are a group of elite individuals who govern the country of Eshar and the people living inside of it. Their leader, Charles Sawyer, will do everything in his power to stop Stephen from finding out the truth behind Eshar, including killing his younger sister, Faith.

As the story continues on, Faith is "poisoned" by the ministry, and Stephen is framed for her death. Faith awakens in the midst of a forest, surrounded by a rugged group of men and women unknown to her. She learns that this group is promptly known as the "survivors" and that they switched the vial of poison the Ministry attempted to use to kill her with a virus that freed her from the government's control over her body and mind. The group is led by a reclusive and introverted man named Quentin Barnes, who wants nothing to do with her. And so the two siblings are split apart, with one being confined to a cell and the other living a sliver of a free life, which is more than they have ever known. Both face their own set of troubles; while Stephen faces torture, interrogation, and guilt, Faith fights for survival, uncovering hidden truths, and her own curiosity, which threatens her life. Stephen learns that before he was even born, his parents were wanted by the Ministry for speaking their minds and searching to see if the world beyond reality truly did exist. Charles Sawyer breaks Stephen down to a skeleton of what he once was, telling him he was responsible for his sister's death and that there is no world beyond reality. Faith, however, comes to find out that the irritable leader of the survivors, Quentin Barnes, was her father's best friend and the one responsible for his death. He informs Faith that even though he turned her father over to the Ministry, her father still trusted him with his most valuable possession: the key to the world beyond reality. He gives Faith the key and makes her promise never to let it out of her sight.

The novel's story arc is wrapped up in a cinematic-like finish when Stephen and Faith are reunited, at the expense of Quentin (who was killed in order to bring the two together), only to learn that the Ministry is still after them and will continue to be unless they make an escape into the world beyond reality. They find their father's last clue hidden in their own backyard, urging them to go to a mysterious location in District Ten, known as "The Dome." The two siblings rush through the city, dodging Esharian guards and Ministry of Information officials, before they run into E721 Ghost, the Ministry's leading assassin who was genetically engineered to kill. Ghost had been after them since the beginning of their adventure and was the one responsible for, among other things, attempting to poison Faith and successfully killing Quentin. Ghost captures the Laings and takes them to Charles Sawyer, who feeds them lies and tears them down to nothing but scared and shivering children. Sawyer leaves, and just as Stephen is about to apologize to his younger sister, the lights flicker off and sirens begin to blare, allowing them an opportunity to escape from the Ministry for one last time. As soon as they reached The Dome, a massive building in the center of District Ten, which houses discarded portal technology possessing the ability to travel to a world outside of their universe. The assassin Ghost appears again and threatens to murder Faith, only to be stopped by Stephen, who shoots him with his own gun. Together, he and his little sister go through the door of the dome, find a way to engage the portal (using their father's key), and step through the doorway into what they believe is the "world beyond reality."
