Description: This is a complete procedure-oriented book on spine injections commonly practiced by pain physicians for diagnostic or therapeutic purposes. It has been updated with three new chapters since the previous edition of 2003.
Purpose: The main purpose is to provide a comprehensive guide to spine injections for practicing or trainee pain physicians. It covers all aspects of injections from patient selection to complications, with a focus on injection technique and how to use fluoroscopy for guidance.
Audience: Useful for any physician who treats pain patients and performs spine injections, the book's audience includes orthopedic surgeons, radiologists, neurosurgeons, and pain anesthesiologists. It provides both basic information and practical tips about various injections, which makes it useful both for trainees and experienced practitioners. The editors and contributing authors are all very experienced clinicians.
Features: The four introductory chapters cover anatomy of the spine, imaging equipment, drug pharmacology, and patient evaluation and selection for the procedures. The chapter on spine anatomy is an excellent review of bone structure and the vascular pattern of bone and nerves - a topic not generally covered by other spine injection books. Chapter four is a wonderful review of the pros and cons of various back injections, also often missing in other books. These are followed by a series of chapters, each of which details an injection technique. Practically every injection is covered, from discography at all levels of the spine, epidural injections, facet injections, dorsal column implants, etc., to kyphoplasty. Autonomic blocks, i.e., stellate ganglion block, lumbar sympathetic block, and celiac plexus block are also discussed. Another chapter covers endovascular therapies to treat different vascular anomalies causing back pain. All chapters have more or less the same format. Each starts with an introduction to provide the background for the injection, then continues with patient selection and indications for the procedure, the step-by-step approach for the injection technique, and how to use image guidance for proper and safe needle placement. Complications are discussed separately and are followed by concluding remarks to recap the chapter. The text is straightforward, there are plenty of images, and, where needed, key points emphasize important material. The book discusses every injection in its clinical context. The idea is to train clinicians, not injectionists. The only subject it slights is radioablative therapy of medial nerve branches, which it mentions briefly, but it deserves more attention. However, the chapter on pulsed radiofrequency of DRG is a treat to read — it has the best description of the technique I have ever seen.
Assessment: There are many books on this subject and each has some merits or demerits. Either they are too short or too cumbersome. However, this book strikes the correct balance, discussing every procedure in a clinical context while providing a step-by-step description of the technique using image guidance. The second edition is needed to cover the emerging new procedures. No physician treating pain patients will regret buying this book.
