About the Author

Erika Claud, MD specializes in neonatology, providing care to critically ill infants. She has an interest in the diagnosis and treatment of preterm infants and conditions of the immature digestive tract. Dr. Claud's research focuses on the role of the intestinal microbiome in preterm infant development and neonatal necrotizing enterocolitis (NEC) as well as on intestinal epithelial biology as it relates to neonatal necrotizing enterocolitis (NEC), a life-threatening inflammatory bowel disorder of unknown cause that afflicts premature infants. Her work has been funded by grants from prominent institutions, including the March of Dimes and the National Institutes of Health (NIH). A dedicated educator and author, Dr. Claud has published several articles and abstracts in peer-reviewed journals and has written chapters in leading textbooks on pediatric gastrointestinal disease. In addition, she serves as an ad hoc reviewer for several scientific journals, such as the Journal of Pediatrics and Gastroenterology.

Description: In 2014, Dr. Martin Blaser sounded the alarm that the overuse of antibiotics was changing the human microbiome in a book aimed at the general public entitled Missing Microbes (Henry Holt and Company). Now Dr. Erika C. Claud and colleagues have written a text for the medical community summarizing the research to date on this incredibly important topic. The first edition of this book focuses on the establishment of the microbiome in early life and the influences of various factors in the environment. This well-written and concise paperback book reviews the evidence that long-term clinical outcomes are influenced by a wide array of events in early life (more than just the administration of antibiotics) that affect the human microbiome.
Purpose: The purpose of the text is to summarize the most recent research on various aspects of the developing microbiome and to propose linkages to health and disease in later life. This is a very worthy objective, which should see an increasing amount of research in the next decade. The book certainly meets this objective, although at this point in the research, it raises more questions than it answers.
Audience: According to the author, the book is intended for researchers and clinical scientists as well as pediatricians and gastroenterologists. However, this is such an important area of new knowledge, it is this reviewer's belief that it should be read by almost all practicing physicians as one of the key areas for wellness in the next generation. The author and contributors are well known in this developing field.
Features: The relatively short text (200 pages) is divided into 10 sections, each with brief reviews about certain topics related to the development of the microbiome. The text is extremely well referenced. Although many sections are related to gastrointestinal issues (bowel disease, food allergy, obesity, vitamin D, etc.), other sections deal with factors that affect the microbiome in the womb, the immune system, neurodevelopment, and later health implications of changes in the microbiome. A final chapter deals with how the gut microbiota may be manipulated. There are few illustrations in the text, but the ones that are used are clear and colorful and help explain the manuscript.
Assessment: This book gives a wonderful review of the research in this quickly evolving and very promising field. The writing is concise and well researched. I doubt it will be a bestseller, but it should be required reading for all pediatric and gastrointestinal residents as well as many others in clinical medicine.
